Consumer Rights, Records Access & Licensure Information

Requesting Your Health Care Records

Clients and former clients have the right to request access to their health care records in accordance with Texas law and the Health Insurance Portability and Accountability Act (HIPAA).

To request copies of your records, please submit a written request that includes your full name, date of birth, dates of service requested, and your preferred method of delivery.

Requests are typically processed within 15 business days unless an extension is permitted by law.

Filing a Consumer Complaint

If you have a concern related to access to health care records or a consumer protection issue, you may file a complaint with the Texas Office of the Attorney General, Consumer Protection Division.

Professional Licensure

Jennifer Miller is a Licensed Clinical Social Worker – Supervisor (LCSW-S) (TX #68737) licensed by the Texas Behavioral Health Executive Council and a Licensed Clinical Social Worker (LCSW) (VA #0904016524) licensed by the Virginia Board of Social Work.

Emergency Notice

This practice does not provide emergency or crisis services.

If you are experiencing a mental health emergency, call 911, go to the nearest emergency room, or contact the Suicide & Crisis Lifeline by calling or texting 988.

Therapy & Clinical Services

The Place Psychological Services, PLLC provides licensed mental health services to adults in accordance with state law.

 

Services include psychotherapy, psychological assessment, and trauma-focused treatment approaches provided within the scope of professional licensure.

Clinical services are available only to individuals physically located in states where the provider is licensed at the time of service.

​

Educational offerings through The Place Academy and Corporate Workshops are separate from clinical services and are not regulated by state licensing boards.

Good Faith Estimate (No Surprises Act)

Under the No Surprises Act, clients who do not have insurance or who choose not to use insurance have the right to receive a Good Faith Estimate of expected charges for mental health services.

The Good Faith Estimate shows the cost of services that are reasonably expected for your care. You may request a Good Faith Estimate before scheduling services or at any time during treatment.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you may dispute the bill.
